MEDICAL RESPITE
The Medical Respite Program provides short-term housing to homeless hospital
patients who are medically cleared for discharge but do not have a stable environment
to complete their recuperation.
All too often people, especially those who are homeless or unstably housed, remain in the hospital long after they are medically cleared for discharge. This situation is not healthy for the
patient or cost efficient for the hospital.Partnering with hospitals and managed care companies, Comunilife’s 80 bed Medical Respite program provides a short-term housing alternative where patients
can complete their recovery. While in the Respite Program, patients access medical care and other supportive services with the goal of reducing preventable re-hospitalizations and Emergency Room visits.
Understanding that permanent housing is the greatest social determinant
of health,Respite’s goal is to break the cycle of shelter to hospital to shelter,
by having staff work with the patients to secure the proper level of permanent/next step housing.
MEDICAL RESPITE RESULTS
The Medical Respite Program programs results speak for themselves. Only 5% of the clients were
readmitted to the hospital or had an Emergency Room presentation within 30 days of transfer to Respite.
85% of the patients, who were discharged to permanent housing, moved to Comunilife supportive
and affordable housing.
