Health & Housing.

A continuum of 3,107 affordable and supportive units — because no one
should return to homelessness for lack of the foundation to succeed.

We believe that housing is a social determinant of health — without a safe, affordable, secure place to live, it is impossible to fully overcome health and social issues. For individuals who have experienced homelessness or long-term hospitalizations, stable housing is often the first step toward a safer, healthier future.

Comunilife meets people where they are. We surround each resident with the services, care, and encouragement they need to stay housed and move forward with confidence — equipping every individual with the tools and stability to live independently and fully in the community they call home.

Family & Youth Services.

An after-school suicide-prevention program for adolescents

The only program of its kind in the country. 

Each day, teenagers participate in group activities that include academic support, expressive arts therapies, health and wellness, and family services. We offer activities that allow teenagers to thrive by finding their unique voice and flourishing, providing them with the tools to visualize and achieve their goals.

Through LIP, the teens have learned to advocate for themselves and their peers through civic participation. To date, more than 750 adolescents have participated and none of them have died by suicide.

An evaluation conducted by the New York State Psychiatric Institute / Columbia University Center of Excellence for Cultural Competence demonstrates that for each month an adolescent participates in LIP, their level of suicidal ideation and depression decreases.

Medical Respite.

Short-term housing for hospital patients medically cleared for
discharge — so people heal in a bed, not on a sidewalk.

All too often, people — especially those who are homeless or unstably housed — remain in the hospital long after they are medically cleared for discharge. This is not healthy for the patient or cost-efficient for the hospital. Comunilife’s 63-bed Medical Respite program offers a short-term housing alternative where patients can complete their recovery.

Partnering with hospitals and managed-care companies, we surround each patient with medical care and supportive services — reducing preventable re-hospitalizations and emergency-room visits. Understanding that permanent housing is the greatest social determinant of health, our staff work with each patient to secure the proper level of permanent or next-step housing, breaking the cycle of shelter-to-hospital-to-shelter.

A discharge destination for medically cleared patients who would otherwise stay in a hospital bed for lack of a safe place to recover. Lower readmissions, lower ER returns, better outcomes.

Program 04 · Three connected services

Community Engagement.

Health Homes Care Coordination, ENGAGE short-term mental-health support, and the Social Care Network — connecting community members to the housing, food, transportation, medical services, and behavioral-health care that shape real outcomes.

Care Coordination Health Home

A Medicaid-funded coordination model for individuals with complex medical, behavioral health, and social service needs — built around six core services: comprehensive care management, care coordination, transitional care, individual and family support, referrals, and care-team communication.

Comunilife ENGAGE

Free, accessible, culturally responsive short-term services for adults experiencing mild to moderate symptoms of depression, anxiety, PTSD, substance- use challenges, and financial stress. In English, Spanish, and French. No insurance needed.

Social Care Network (SCN)

A statewide program for NYS Medicaid members. Navigation and enhanced services for those facing homelessness, eviction risk, behavioral health challenges, recent hospital or carceral release, or perinatal vulnerability.
