Project Coordinator – Pathway to Community Living – Bronx, NY

Comunilife is a premier provider of housing, mental health, and HIV/AIDS services in New York City.  Our programs benefit more than 2,500 people each year – helping  them overcome challenges related to extreme poverty; chronic homelessness; serious behavioral and medical issues; and, linguistic and acculturation barriers.

Our Pathway to Community Living program, funded by the Office of Temporary and Disability Assistance (OTDA), provides mental health and after care for HIV/AIDS clients with mental health needs. Services include mental health assessments and referrals, independent living skills, peer mentoring, and after-care when relocated to permanent housing.

We are looking for a Project Coordinator to assume responsibility for all aspects of the peer mentoring program. Responsibilities include:

  • Day to day operation of the Pathway to Community Living Program
  • Assisting in the integration of the Pathway to Community Living Program into the Westchester and Crotona North Residences
  • Documenting, tracking and evaluating program activities and generating required reports
  • Conducting outreach and recruiting peer mentors.
  • Development and implementation of training for peer mentors
  • Assigning peer mentors to residents and graduates and accompanying peer mentors on their first visit to residents
  • Providing ongoing individual and group supervision of peer mentors.
  • Providing supervision to the Mental Health Counselor
  • Participating in case conferences, trainings, workshops and seminars as well as meetings with peers and supervisors as required.
  • Collaborating with other program staff in providing client services.
  • Participating in weekly supervision.
  • Other duties as assigned.

After-care responsibilities include:

  • Referrals, advocacy and monitoring of health, mental health, substance abuse, and any other needed services
  • Home visits

Knowledge and Skills:

  • Extensive knowledge of community mental health services and other resources.
  • Knowledge of counseling processes and practices.
  • Ability to learn HRA Supportive Housing Program Desk Guide and the HIV/AIDS Program regulations.
  • Responsible, organized, effective, and efficient.
  • Ability to perform multiple assignments simultaneously.
  • Excellent communication skills (verbal, written and active listening)
  • Community minded, patient, creative, flexible, compassionate, and culturally sensitive to persons who are diagnosed with HIV/AIDS, and severe and persistent mental illness.
  • Ability to deal tactfully and diplomatically with other Comunilife employees, Federal, State, and City officials, professional and technical groups, clients, and the general public.

Requirements:

  • LMSW with experience working with populations with HIV/AIDS and mental health issues.  At least two years supervisory experience and two years training experience.
  • Experience working with issues of homelessness, substance abuse and mental health. Bilingual Spanish preferred.
