In-Take Coordinator – Outpatient Mental Health Clinic: Vida Guidance Center – Bronx, NY

The Vida Guidance Center, a licensed outpatient Mental Health Clinic, provides bilingual bicultural services to children, adolescents, adults and seniors in the Bronx diagnosed with mental illness, chemical addiction (MICA), and HIV/AIDS.  Services include family, individual and group psychotherapy, psychiatric evaluations, counseling, medication follow-up care, and case management.  Clinic patients are primarily Latino and African-American.

We seek an Intake Coordinator who will:

  • Complete brief telephone assessment to determine appropriateness of referral to the clinic; perform initial screening for risk assessments
  • Respond to COPS referrals in a timely manner, and schedule intake appointments
  • Complete initial referral information and financial information forms and initial insurance verification and authorization
  • Keep appropriate documentation of all records, including cases not accepted for treatment
  • Carry cases until they are assigned to a therapist
  • Assess client needs and make referrals to community resources as needed
  • Arrange intake evaluation and psychiatric evaluation appointments and monitors waiting list for follow up
  • Track referrals, intake evaluations, and psychiatric evaluations for timely scheduling according to OMH guidelines
  • Support clinical staff by making referrals and follow up as needed
  • Follow up insurance authorization, as needed

Qualifications:

  • MSW, and New York State LMSW required.  Experience of working with SPMI, MICA, and HIV/AIDS afflicted adult population.  Bilingual in Spanish/English.
  • Extensive knowledge of principles and practices of mental health outpatient clinics and Office of Mental Health regulations and directives.
  • Ability to provide intake coordination, case management and counseling to clients diagnosed with severe and persistent mental illness and/or histories of substance abuse and trauma.
  • Responsible, organized, and efficient.
  • Ability to perform in a team approach and carry out multiple assignments simultaneously.
  • Excellent communication skills.
  • Community minded, patient, creative, flexible, compassionate, and culturally sensitive to persons who are diagnosed with severe and persistent mental illness.

This is a full-time position.  Some evening and weekend hours are required.
