Social and Medical Respite

Comunilife’s Social and Medical Respite Program is a short-term residential program for patients who no longer require hospitalization but do not have suitable housing in which to convalesce. While in our Respite Program, patients are still able to access medical care and other supportive services.

The Medical Respite Program provides:

  • Safe and sanitary, smoke-free room and board at the Respite Facility with 24-hour supervision and monitoring, building security and housekeeping,
  • Personal hygiene items and access to laundry facilities,
  • Case management services that include arrangements for follow-up medical care, social service assistance with transportation to follow-up medical appointments, and placement in appropriate housing following program discharge.

Our goal is to reduce preventable re-hospitalizations and Emergency Room visits.
